That couldn't be Tom, it's too early. In the above two examples, both statements are set in the present tense, but the first one is a definite statement of fact, while the second statement is based on the implied impossibility of something, in other words, conjecture.

The sentence (A) isn't correct grammatically. "I couldn't open the door" is in the past. As a matter of fact, you are locked in and unable to open the door at the present time. So the correct sentence should be: I am locked in here and cannot open the door. Can you help me? The second sentence (B), which is in the past, is correct grammatically. Could is the past of can.

The test-setter is wrong. We don't use couldn't help for things that you have to do for a good reason. It is used, as you say, for things you can't stop yourself from doing (laughing at something funny, eating more of a delicious food) and also for things you do by accident (overhearing a remark that was meant to be private, for example).
